This lecture will explore different types of SARDs, antibodies that may occur in SARDs, common methods of autoantibody testing and appropriate autoantibody testing in SARDs. Beginning with an introduction to immunity and autoimmune diseases, we then discuss single and multiple organ involvement in autoimmune diseases. We will look at Grave’s disease, Primary biliary cirrhosis, pernicious anemia and myasthenia gravis. Turning to SARDs, we discuss autoantibody testing, including tests for antinuclear antibodies (ANA), Rheumatoid Factor, anti-double strand-DNA antibodies, anti-Smith antibodies, anti-Histone antibodies, SS-A and SS-B, Scl-70 and anti-Jo-1 antibodies. Next we will discuss how additional information for a potential diagnosis can be provided by the testing for the presence of antibodies targeted to extractable nuclear antigens (ENA). We look at common methods of testing for antibodies and how to detect antibodies that have bound to the antigen, including indirect immunofluorescence (IIF), enzyme linked immunosorbent assay (ELISA), and multiplex bead immunoassay (MPBI). Finally, this lecture will address the clinical utility of autoantibody tests in SARDs.
